We invite you to become a Member of YOUR Community Health Service.
Why be a member?
- Being able to have a say in the future of Mitchell CHS including planning, identifying service gaps and shaping new and existing programs and services
- Representing your community
- Being invited to participate in focus groups and public forums to improve our services
- Eligibility to be on the Board of Management
Who can be a member?
The following criteria applies:
You must be:
- 18 years of age, or over
- Reside, study or work in areas serviced by Mitchell CHS
- Be a client of the agency (or a carer of a client of the agency), if you live outside of the area serviced by Mitchell CHS
- Be a volunteer of Mitchell CHS
What does membership cost?
Being a member of Mitchell CHS is free.
How do I become a member?
Complete and submit the Membership Registration Form or down load and return to us either in person, or by mailing to PO Box 84, Broadford, 3658.
