HACC Assessment Officer

Do you have a tertiary qualification in nursing, allied health or counselling?

Mitchell Community Health Service (MCHS) is looking for someone to assess and coordinate home based services for frail, aged and younger people with disabilities residing in Mitchell Shire. This is a 0.8 EFT position.

You will be part of a model of care and service provision where MCHS will be working with clients to remain living independently in their own homes.

Knowledge of the HACC program and experience in assessment, care planning and care management is essential.
