Intake and Initial Assessment

Referrals are welcome from any source, including self referral. 

Prior to receiving services, clients will be provided with a comprehensive and holistic assessment to determine individual needs and the range of services available to remain living independently at home.  The assessment may include:

• Initial needs identification
• Functional assessment
• Falls risk assessment
• Nutritional assessment
• Home safety check
• Discuss referrals to other services
