Disability Access & Information

Mitchell Community Health provide the following Disability Access services:

Disability Case Management

Disability Case Management is for people aged 16 to 65 years who have a physical or sensory disability or ABI and who reside in Mitchell and Murrindindi Shires.
The program operates on an outreach basis - home and community visits can be made by arrangement. The service provided can be flexible and individualised to suit you.
The goal of the Disability Case Management program is to:

Improve quality of life and facilitate the provision of services that are tailored to meet individual need. 

The emphasis in case management is on partnership. We don’t manage clients but seek to work collaboratively with and for the client. The role is one which supports coordinated continuity of care and seeks to address fragmentation within a rural environment.

  • Assessment
  • Consultancy
  • Case Management and planning
  • Information & Referral
  • Support, linking, monitoring
  • Counselling
  • Advocacy

How do I access the service?

The service is available fivedays per week during business hours.

Rural Access

The goal of Rural Access is to build a more inclusive community so that disability is no longer a barrier to participation in community life. The focus is on working in partnership with the community, service providers, people with disabilities, their carers and friends to increase the opportunities available for people with disabilities to participate in community life.

The Rural Access worker aims to promote the inclusion of people with disabilities, through such things as:

    • Improving community understanding about the needs and barriers of people with disabilities in their community.
    • Develop and implement projects which respond to needs and priorities.
    • Ensure that the needs of people with disabilities are included in community planning.
    • Developing easier ways for people to access information they need about services, community activities and issues.

          How do I access the service?

The service is available five days per week during business hours.

Rural Access works across all groups and sectors to increase the participation of people with disabilities in their community. The program works with all disabilities and ages.
