Mitchell Community Health provides the following Nursing services:

Community Health Nursing

The Community Health Nurse acts as a contact person/facilitator for a variety of programs including:

  • Diabetes Support  Groups
  • ‘Parenting Your Aged Parent’ support group
  • RU12 (Gay, Lesbian community group)
  • Walk & Talk groups ( Seymour , Broadford, Wallan, Wandong)
  • Wednesday Ladies Social Group
  • Quit Freshstart and Short courses

Individual services including:

  • Diabetes resource person
  • Health Promotion including
  • Community screening
  • Community Education sessions
  • Private consultation in regard to disease processes, medication and management
  • Human development programs for local schools
  • Asthma Educator
  • Smoking cessation services
  • As well as ongoing, needs driven services .

Continence Nurse Advisor

The Mitchell CHS Continence service is funded via the Home and Community Care (HACC) program. This program provides services to frail older people, people with disabilities and their carers.

The Continence Nurse Advisor provides:

  • Assessment, advice and management strategies to clients and caregivers regarding bladder and bowel control in the home.
  • Provides information and education on the use of aids and appliances and how the client, carer or health professionals can access these.
  • Provides education and information on specific and general continence issues to various health professionals, clients and caregivers.

District Nursing

The District Nursing Service is part of the total health care delivery system providing nursing support services to people in their home environment.

The main objective of the service is to ensure that high quality professional nursing care   remains an option for people of all ages, who choose to return to or remain in their own homes.

The District Nurse works closely with General Practitioners, Physiotherapists, Occupational Therapists, and Dieticians etc.

What Do District Nurses Do?

District Nurses focus on the individual needs of each client and carer to:

  • Assess their nursing needs
  • Plan and provide nursing care which includes
  • Technical Procedures
  • General nursing care
  • Rehabilitative care
  • Support
  • Health Advice and Education
  • Palliative Care
  • Review and monitor progress
  • Referral to other services where required e.g. Home Care, Meals on Wheels

            Aged Services information

Well Women’s service

The Mitchell CHS Well Women’s Service offers women of all ages the opportunity to discuss issues relating to the maintenance of good health and well being.

The Well Women’s Service is conducted by experienced Registered Nurses (Div 1) who have completed specialised training with
Family Planning Australia in sexual and reproductive health. They are governed by standards of practice and are required to
participate in ongoing education to maintain expertise.  This also entitles them to be registered as Nurse Pap Smear test providers.

Women’s Health Nurses have a commitment to providing health care specific to women’s individual needs.

Privacy and confidentiality is assured at all times.

What type of services do Women’s Health Nurses provide:

Women’s Health Nurses understand that there are many social, emotional and cultural issues that can affect people’s health.  We are committed to offering services that meet individual needs.  The services available from a nurse-based clinic include:

  • Pap tests
  • Breast self examination instruction
  • Pregnancy Tests

Information and time to discuss your own health  issues, which may include:

  • Pap Smear results
  • Menstruation
  • Contraception
  • Pregnancy
  • Breast Health
  • Sexuality
  • Endometriosis
  • Incontinence
  • Hormone Replacement Therapy
  • S.T.D.’s
  • Menopause
  • Any other issue

Why would I want to see a Women’s Health Nurse:

Women’s Health Nurses work as part of a health care team.  Each member of this team, such as doctors,  podiatrists and  physiotherapists, have particular skills.  Often people prefer to see a nurse because they appreciate the extra time a nurse provides to discuss questions and explore issues in more detail.  Some women simply prefer seeing another woman for their sexual and reproductive health care.

What else can a Women’s Health Nurse provide:

Women’s Health Nurses have a broad range of skills, which we share with our community.  Our work often involves us in health awareness campaigns, educational talks, participation in community development projects and support of local initiatives.

Referrals and Service Fees

Well Women’s Service

  • No referrals are required to attend the Well Women’s service.
  • There is no fee for using this service.

District Nursing Service

  • Referrals are accepted from any source, including individuals, families, carers, general practitioners, hospitals and other service providers.
  • A doctor’s referral is necessary for the management of medications and specific medical conditions.
  • The current rates per visit are available on request from your district nurse.

Community Health Nursing and Continence Nursing

  • Referrals to the Continence service can be made by Health professionals, carers, family members and the client. Referrals are accepted if the client fits the HACC criteria and the client has given consent for the service (in some instances the client may be unable to make these decisions).
  • Cost per visit $6.50 for pensioners, health care card holders and people on low incomes.  All others $10.00 per visit.
  • No-one will be denied service because of an inability to pay.
